Membership & Fees
Membership is an annual fee of
$2,800
which can be paid annually, semi-annually, or quarterly.
Your insurance doesn't go away. We continue to bill Medicare and the commercial plans we accept for routine and sick visits, just as before.
Adult membership includes care of your children aged 5-24 under standard commercial insurance fees. They do not require a separate membership.
The membership fee covers preventive services that insurance typically doesn't reimburse.
While the fee itself isn't reimbursable by insurance, it may be eligible for payment through an HSA or FSA-check with your plan administrator.

Dr. Ellenbecker offers a complimentary meet-and-greet to prospective patients (and significant others). This gives you the chance to ask questions, share your health goals, and decide whether his practice is a good fit for your needs.
